Circle of Care is an independent non-profit organization dedicated to assisting Passaic County youth (up to age 21) and their families. Our goal is to help families attain positive outcomes for their children in their own homes and communities.

What to Expect After Referral

Your family is never alone in this process. Once enrolled, you’ll be connected with a Child-Family Team (CFT)—a dedicated group of people who work together to support your child and family. This team includes you, your child, your Care Manager, and both natural supports (like family, friends, and neighbors) and formal supports (like teachers, counselors, and therapists).

Together, the CFT helps build a plan that reflects your child’s strengths and needs, and ensures your family has the right support in place at the right time.

What the Child Family Team Does

The CFT works with your family—not for or around you. Here’s what you can expect:

  • Meets Regularly
    The team meets on a regular basis to check in, review progress, and adjust the plan as needed.
  • Develops a Personalized Plan
    Collaboratively creates an Individual Service Plan (ISP) based on your child’s unique needs, goals, and strengths.
  • Coordinates Services and Supports
    Helps connect your family to in-home supports, therapy, community programs, school services, medical care, and more.
  • Responds to Challenges
    Supports you during times of crisis or change, updating safety plans or identifying new strategies as needed.
  • Empowers Your Family
    Builds up your family’s confidence and self-sufficiency, helping you gain the tools, resources, and voice to navigate systems on your own.

The CFT grows and evolves with your family—always centered on what matters most to you.

What the Action Plan Can Include:

  • In-Home Therapeutic Supports: Personalized counseling and skill-building services within the home environment.
  • Linkage to Community Resources: Connecting to youth programs, recreational activities, and support groups.
  • Advocacy and Educational Support: Assistance navigating school systems, special education services, and accommodations.
  • Crisis Intervention and Safety Planning: Developing strategies to ensure your child’s safety and well-being.
  • Healthcare and Medical Coordination: Access to mental health services, developmental assessments, and medical care.

Our goal is to create a comprehensive support system that addresses your child’s emotional, social, educational, and health needs, helping your family move forward with confidence.

Circle of Care services are community based, individually tailored to children and their families, flexible, and outcome based.

Circle of Care endeavors to be culturally competent, with services that are responsive to the culture, race, and ethnicity of the families we serve and the communities in which they live.
